PlaceWell is staffed by experienced commissioners and clinicians who validate every referral before matching. We focus on outcomes — not provider listings.

    Submit referral

    Placement teams complete the secure referral form, attaching needs assessments, OT reports and risk information. A unique reference number is issued immediately for tracking.

  2. 02

    Review queue

    Each referral enters a triage queue. The PlaceWell team reviews requirements within one working day to confirm completeness and assess urgency.

  3. 03

    Requirements assessment

    A Hub administrator assesses housing, care and clinical requirements against the individual's preferred area and exclusion criteria.

  4. 04

    Matching

    Suitable housing associations and care providers are identified from PlaceWell's vetted national network — including specialism, capacity and CQC profile.

  5. 05

    Proposed options

    The placement team receives proposed options with full provider information so the local authority can make its decision.

  6. 06

    Outcome tracking

    Once a placement is agreed, PlaceWell tracks the placement through transition and records outcomes for evidence and learning.
